MONROVIA – The Liberia National Police has formally charged Samuel P. Jackson in connection with the tragic death of a South African national, N-tom-bee-keh-yee-seh Innocentia Khumalo, widely known as Toni, following what authorities describe as a comprehensive and evidence-driven investigation.

Speaking at a press briefing on Thursday, April 2, 2026, at the police headquarters in Monrovia, Inspector General Gregory Coleman addressed the nation, emphasizing the gravity of the case and the need for professionalism and adherence to the rule of law.

“We gather today under solemn circumstances,” Coleman said, extending condolences to the family of the deceased and the people of South Africa. “This is a moment that demands not only empathy, but clarity, discipline, and fidelity to the rule of law.”

According to the police chief, the investigation began immediately after the case was reported, involving multiple specialized units, including the Crime Against Persons Unit and the Forensic Section, in collaboration with medical authorities and the Montserrado County Coroner.

Coleman stressed that every step of the inquiry was guided by evidence and professional standards, leading to what he described as a “body of evidence that cannot be ignored.”

Among the key findings, investigators uncovered what they termed a pattern of prior violence against the victim. “There is credible and corroborated evidence indicating that the victim was subjected to repeated physical violence over time,” Coleman revealed, citing witness testimonies, photographic records, and documented communications.

The police also established that Jackson was the last person seen with the victim in a secured residence and the first to discover her unconscious. Authorities noted that the environment was tightly controlled, with restricted entry and exit, effectively isolating the victim at the time of the incident.

Forensic and medical examinations provided critical insight into the nature of the injuries sustained. According to Coleman, the victim suffered multiple contusions, lacerations, and hemorrhaging, particularly to the head and chest, along with bruising that may indicate defensive injuries or restraint.

“These findings establish a clear trauma profile,” Coleman stated, explaining that the injuries suggest a sequence in which a blunt force impact caused the victim to fall backward, resulting in severe head trauma. “This is not a random or isolated injury,” he added.

Investigators further cited evidence of neglect, noting that the victim had a known medical condition but was left alone in a locked environment without supervision. “This constitutes a serious breach of duty of care, with foreseeable risk to life,” Coleman said.

The police also highlighted post-incident conduct as part of their findings, including the alleged failure to promptly notify authorities and indications that the scene may have been altered before officers arrived.

While the investigation continues, including pending toxicology results and a full autopsy report, the LNP confirmed that Jackson has been formally charged with manslaughter, negligent homicide, aggravated assault, and reckless endangerment under Liberia’s penal law.

“Let me be unequivocal,” Coleman emphasized. “The Liberia National Police does not determine guilt; we act when the evidence meets the legal threshold. The accused remains presumed innocent until proven guilty in a court of law.”

Beyond the immediate case, Coleman used the moment to send a broader message, warning that violence within private spaces will not be tolerated and that neglect leading to harm carries criminal consequences.

The police assured both the Liberian public and the international community that the case has been handled with professionalism, transparency, and respect for international standards.
